School Access Form
In an ongoing endeavor to provide safety and security for students and staff, school access is REQUIRED for every individual that is not a staff member of the St. Johns County School District. If a person does not have school access, they cannot go past the front desk into the school.
If you have previously filled out the form for another school in St. Johns County School District, it will automatically be transferred. School access lasts for 3-year intervals.
Upon approaching 3 years of access, it is the responsibility of the volunteer to contact the receptionist at Pine Island Academy to renew the form. To reduce duplication in the system, please do not complete a new form.
Always Remember! Proper identification (current state issued driver's license or current state issued identification card) must be presented each time a person signs in at school. If a person has a visa and/or doesn't have a social security number, the applicant must contact the Volunteer Services department at (904) 547-3952 for alternate steps.
